JOB TITLE: Care Coordinator

ACCOUNTABLE TO: Manager and Director

Purpose of the position

  • To assist in the management of services
  • To maintain continuity of standard of services
  • To report any concerns

Scope of position

The Care Coordinator provides and Coordinates individualised care and resources to elders, disabled and others so that they may continue to live independently as much; in their own homes and in the community.

The Care Coordinator is also responsible for managing and administering the services of which Precious Care Services Ltd provides. The Care Coordinator will maintain confidential Service User files and information at all times, in accordance to Policy. The Care Coordinator will prepare and present reports and information as required. The Care Coordinator will manage and distribute resources effectively, and will ensure that all care is provided in a caring and respectful way in keeping with all relevant Policies and procedures and Precious Care Services Ltd standards.

The Care Coordinator will liaise with other community services and resources to ensure that other resources are involved, if required. The Care Coordinator will also provide information and education to individuals and groups in the community, as required and requested.

Failure to provide adequate services may put elders and others at risk and /or force elders to move from their home before they are prepared to.

Responsibilities

1. Administer Precious Care Services in order to ensure the services are delivered in an appropriately, caring and respectful manner.

Main Activities;

  • Ensure care provided is according to all relevant policies, procedures and regulations
  • Monitor and evaluate all tasks of the Care Worker
  • Monitor supplies and resources
  • Identify persons requiring home care and Service User needs
  • Make recommendations for changes and improvements to the program, as required
  • Schedule work assignments with regard to Precious Care Services training and experience
  • Collection of monies.

2. Coordinate with Management

Main Activities;

  • Ensure all Service Users needing care or changes to care are identified
  • Coordinate appropriate care and equipment including other community resources, as required
  • Establish and maintain an accurate, confidential Service
  • Provide information to other health care professionals, as required
  • Consult with family members and other supporting agency to ensure that care is on-going and that all client needs are identified and met.
  • Discuss any concerns with management

3. Provide individualised Care

Main Activities;

  • Provide care to elders, disabled persons, and other in the community with identified needs
  • Ensure Care Plan information is gathered, completed and reviewed accordingly
  • Provide care including household hygiene, personal care, administrative care and social care requirements
  • Assist with exercises, physiotherapy and other medical plans
  • Follow medical instructions carefully and consistently
  • Organise time and resources based on the individual needs of the Service User
  • Manage and unanticipated events or unstable situations

4. Coordinate community resources and supports

Main Activities;

  • Encourage Service Users and families to be involved in the care they receive, if appropriate
  • Main point of contact
  • Encourage Service Users and families to take responsibility for care, if appropriate
  • Liaise with all family, medical and other resources, if required
  • Advocate on behalf of Service Users for additional services and resources, as required
  • Establish and maintain current, accurate, confidential files for each Service User
  • Educate Service Users, families and support systems about Precious Care Services or direct to management

5. Perform other related duties
